Medication declaration:
For some medication you need a medication declaration if you’re going on holiday to a country abroad.
If you need a medication declaration depends on the medication you’re using, the country you’re traveling to and the coyntries you come across during the trip and the length of your stay abroad.
In general, a medication declaration is mandatory if medication falls under the Nederlandse Opiumwet.
There are three types of declaration:
- The Schengen medication declaration
- The English medication declaration
- The medication declaration for Turkey
On the CAK website explains the differences and how to obtain a medication declaration. You can also fill in the Medicijnen mee op reis-hulp (medication on travel aid) for tailored advice.
Please note: After you filled in the Medicijnen op reis-hulp, it must be provided with the BIG-registration number and the original signature of your GP. Please bring the form or forms to the practice so your GP can sign the declaration.
